The employee was working on and under a dock plate being held up by a forklift.

STRUCK BY MATERIAL EQUIPMENT - Employee (Building Equipment Mechanic) was repairing a dock plate. The dock plate was being held up with the forks of a forklift. A co-worker (Maintenance Mechanic) was operating the forklift. The BEM asked the MM to lower the forks of the forklift. While the MM was lowering the forks of the forklift the dock plate slipped off of the forks of the forklift and the dock plate fell and hit the BEM on the top of the head knocking him to the ground. EMS was called and transported the BEM to Moses Cone Hospital in [REDACTED] for evaluation and treatment.

Head (skull) brain cranial nerves and cerebral blood vessels-Center- Wounding |Neck including spine and cervical vertebrae-Center- Fracture

Dock plate fell off fork lift and struck employee on the head wounding head and fracturing T4 vertebrae

Struck on head head wounded requiring staples when dock plate slipped off the forks of a forklift. Employee also suffered T4 vertebrae fracture.
