On [REDACTED] around [REDACTED] carrier and union [REDACTED] approached me together carrier then reported to me that the same pain that she was experiencing on [REDACTED] is happening again but this time the pain is on her right side pelvis to her hip down to the shin of her leg and that she took an ibuprofen to help with the pain she said the ibuprofen worked a little bit but the pain came back. When she was done explaining herself about the pain she was experiencing carrier requested a [REDACTED] and then union [REDACTED] was telling me she needs a [REDACTED] printed out and that this happened on the job. So I let carrier and union [REDACTED] know that I’m not able to print out a [REDACTED] there is a new process with filling out a [REDACTED] and a [REDACTED]-2 you would have to go on e-comp and fill out the form there. I then reported this to the postmaster after explaining to carrier and union [REDACTED] about filling a [REDACTED]. Then I brought to carrier 's awareness that
