What happened
Employee reached near kitchen sink for spray bottle of cleaner to clean the lunchroom tables. Lid must have been loose as it dropped into sink. Chemical mixture came out of bottle and went all over the employee. Into her left eye mouth and nose. Used eye wash station immediately for 25 minutes.
