Before the incident
The employee was entering the confined warehouse area to communicate that she wanted to get ready for a sort coming up. The employee was entering through a door then around a corner with a walkway.
What happened
The employee entered the warehouse area turned the corner of the walkway where a step stool was located in the middle of the walkway. The employee tripped on the step stool with left foot she then fell forward with her right arm extended over her head.
Injury or illness
The employee fell on their right side without any body part being able to lessen the fall. The employee had a: closed nondiscplaced fracture of proximal end of right humerous; sequela and acute pain of right wrist. Employee required right arm shoulder surgery.
Object or substance involved
The cement floor impact physically making contact with the employee from the trip and fall harmed the employee. The trip over the step stool causing the event to conspire.
