What happened
The employee walked off the main aisle and cut in between [REDACTED] and [REDACTED] where employee slipped on oil water residue that had been caused by a water leak on a long holiday weekend. There were wet floor signs and several cleanings that had been completed since [REDACTED] in the area of the water leak. The employee slipped on the residue hitting the right arm on the dust collector during the fall to the floor.
