Employee was loading a trailer.

Employee was operating a fork truck on the dock and drove over a slip sheet (thin sheet of heavy duty paperboard material placed underneath loads to prevent product from sitting directly on a pallet). This pulled the slip sheet up into the lift. As he was driving up the aisle his steering locked up and jerked his lift in the direction of the racking pinning his right leg and pelvic area between the rack and the fork truck.
