Before the incident
TM is a riding jack driver at debone. From camera footage it showed his [REDACTED] was stationed by the wall of the [REDACTED] area. He came and got on the jack. He appeared to be setting his personal code in the jack.
What happened
The employee is a riding pallet jack driver in our Debone plant. From camera footage it showed that his jack was stationed by the wall of pack out area. The jack was approximately 18 inches from the wall. This operator moved the handle of the jack up and down while standing on the floor between the pallet jack and the wall. On the second time of moving the handle up and down the jack moved on him causing the injury. He had to turn the throttle with his hands to move the jack toward him.
