Before the incident
Employee was manually picking a Customer Order from a ground-level rack position in [REDACTED] 27 of the Warehouse.
What happened
Employee was picking customer orders in the Vault in [REDACTED] at around [REDACTED]. Based on camera footage (CCTV Security Camera) the Employee is seen picking up cases from a pallet at floor-level and as he placed a picked-up box on the empty pallet he stretched out in discomfort. When the Supervisor went into the Vault for a status check she noticed the Employee sitting on a pallet. The Supervisor asked him what was going on and the Employee stated that he had some discomfort in his back. The Supervisor asked him if he needed for an ambulance to be called and he said 'no ' and added that he just wanted the rest his back asking to take off (go home) for the rest of the work day.
Injury or illness
On the way home from work on the same day of the injury the employee did stop to be seen at an Immediate Care Clinic (not at a Hospital Emergency Room (so not at an E.R.)) and was diagnosed with 'Acute Midline Low-Back Pain Muscle Strain Spasm '. No lost time nor restricted time was ordered prescribed by the clinic. Prescription medication was ordered however for pain management for muscle spams to be taken if when as needed. Employee returned to work at his next scheduled work day.
Object or substance involved
Improper lifting technique from poor lifting posture and twisting at the same time as lifting. The employee was picking up boxes cases of product from the floor-level without proper bending of knees bringing box case close to body torso and performing the actual power lift with the legs and without twisting while lifting.
