What happened
Employee was removing an electrical cord from a blower on the GAW platform when he fell to the floor due to an unlocked gate located in the catwalk guard rail. The gate opened causing the employee to fall thru and landed on his right knee and head causing a contusion laceration on his forehead. The employee received a medical treatment of four stitches offsite and returned to work with no restriction.
