Before the incident
I was transferring a patient with PT [REDACTED]. The pt was depended of 2 (Max A of 2). After make sure the pt was in chair and check ROM. I went to the nurses station tto reported RN about the eval and squat down to stretch out my back. My back was hurting due to the transfer. After stretching out my back and stood slowly. When I stood up I could not hear and started to have double vision blur. Then I has blur vision and I started to fall backwards. CNA and PT caught me.
