What happened

Employee was cleaning a 4 deep trench around #18 trimmer . Once the employee had finished the task the employee forgot to reinstall part of the trench cover .Hours latter a separate employee was attempting to remove a jam from #18 trimmer did not see the piece of missing trench cover and stepped into the 4 trench .The employee reported his hurt back to the Forman. The missing trench cove piece was immediately put on . Latter in the day the employee decided to go to a chiropractor since their back was still sore. The employee received an adjustment from the chiropractor .The employee returned to work with no prescriptions and no restrictions.
