Before the incident
Employee was setting up machine for use and was feeding material through the tooling die.
What happened
The employee failed to relieve air pressure from the machine during set up which provides pressure to a sliding tooling. As the material was being jogged the material snagged on the die causing it to slide toward the resisting pressure. The employee placed his hand near a hazardous pinch point while attempting to use a provided tool to align the material through the track of the die. When the material passed through the track the resisting air pressure shifted the sliding tooling back to its original position pinching his finger causing a laceration.

Laceration and minor fracture to left index finger later resulting in an outpatient procedure.
