Before the incident
Broaching armature on [REDACTED] broach machine.
What happened
While broaching armatures on [REDACTED] the operator stuck her finger between the broach and the Armature being broached after actuating the cycle with the 2 handed safety switch. This was done within the 1.5 sec cycle of the broach. The operator reported this resulted from adjusting oil lines during the cycle though through review we determined this could not have caused the accident. The operator did not give additional details on what caused this incident.
Injury or illness
An avulsion of the left index finger. The operator was driven to the emergency room of Midwest Hand Surgery SC for treatment. We were told the injury was cosmetic not functional. The operator missed 9 days of work and was on restricted duty until 9 11 24.
