What happened
Shamrock employee was on the roof clamping the beam down. When he went to tack weld it to the beam the clamp popped off the beam hit the rubber roof bounced 3 times and went through a small opening on the roof (a column penetrating the roof had an opening around it about a 3” gap between column and existing roof). [REDACTED] was working on the mezzanine in the building on the floor below the opening that the clamp fell through. There was approx. 10’ of horizontal difference between the opening that the clamp fell through and the area [REDACTED] was working
