What happened
Employee was in the unpacking line attempting to move a pallet by hand employee was instructed by forklift operator that he would move the pallet by machine. Employee did not listen and when he went to move the pallet he was in the blind spot of the operator. The pallet that the employee was lifting was struck by the pallet on the forklift causing the employee to be pushed backwards causing the employee to fall onto the pallet.
