Staff member was asked to assist a patient back to bed who normally stands and pivots from the chair. Gate belt applied for safety and 2 staff members were present. Patient stood then said I can 't anymore and started to bend her knees. [REDACTED] and staff member tried to assist the person back to her chair but she just became limp. Staff member was able to ease her to the floor and placed a pillow under her head while staff member went to get a lift to get her back into bed. Injured lumbar back and left shoulder while easing patient to the floor. Supervisor was notified.
