Before the incident
STRAIN - EE was removing labels individually from a stack of 4 knockdown totes that were at ground level. EE removed the labels from the sides of the lower two totes then climbed up the stack of totes (rather than using the forklift to drop the stack down)to remove the labels from the top two totes on the stack. As he was climbing down the side of the stack the EE either lost his footing or his foot got caught in the cavity of the tote causing him to fall approximately 3’-4’ to ground level. As he was falling the small of his back impacted a 5-gallon bucket on the ground in the area causing his head to extend backwards impacting the floor.
