Setting up the slug header
What happened
On [REDACTED] at approximately [REDACTED] slug header operator suffered an open end fracture while attempting to remove a jammed bolt in the slug header die. The operator first attempted to unjam it by pressing the K.O. pin button which mechanically pushes up jammed bolts from the die. When the K.O. pin did not work they proceeded to use a wrench to secure the jammed bolt and strike the handle of the wrench to generate enough force to unjam it. However during this process operator 's hand slipped causing them to strike their hand instead of the wrench handle.
