Before the incident
EE was raising a refridgerator to the 4th level in a rack with a forklift. As EE lifted the frdge he heard a loud pop and the hose of the clamp exploded sending the metal pulley at him striking him in the right chest shoulder area.

The employee was on a forklift and was lifting a fridge up to a rack and all the sudden there was a loud pop and a pully flew a the employee and hit him in the chest and states that the wind was knocked out of him.
