What happened
EE was unloading a HSLLP carriage from a batch booth cart. EE Put the hoist on the bottom side of the carriage. As EE was lifting up the carriage rotated and caught the batch booth cart load bar. EE kept lifting until the load bar came off one side and hit EE in the forehead.
