What happened

Tube Bender 5 - setting up for part no. 8897800. Employee was verifying how the tube was to be placed into the bender while the trainer was retrieving the safety tube plug. Employee started the set up process while his trainer was still in process of plug retrieval. Employee was able to slide the tube onto the die and his hand was over the tube and inside the pinch point of the die. The employee then pressed the clamp button several times and it came in contact with his hand. At this time his trainer turned around and quickly released the die.
