walking into room due to pt taking tele off and monitor alarming
What happened
Nurse was at desk charting. Tele monitor was going off. i looked and seen pt took off tele and pulse ox. I walked into room to see what was going on and pt was standing at bedside with gown and brief off yelling for his wife. [REDACTED] started to sway back and nurse went to keep pt from falling. Nurse took a step towards pt and slipped and landed on back. [REDACTED] had peed on the floor.
