What happened
Operator was cleaning the die toward the end of the shift. Maintenance was on the roof working on a ventilation fan. Maintenance fixed the fan and turned it on. Dust and dirt build up was blown off the fan blades and came down on top of the operator while she was cleaning the die. Operator got dirt dust in her left eye
