Before the incident
Line Operator (injured employee) was pulling the tape rope out of 3-inch pipe downstream of the puller.
What happened
As he stepped back and away from the pipe and into the end of a piece of scrap pipe approximately 10-12 feet long. He tripped over it and fell backwards onto the floor landing on his back and left elbow. He was wearing his hardhat which prevented him from receiving a head injury but did slightly injure his neck.
