Before the incident
Grinder operator was attempting to install an extension on the bottom of the grinder shute.
What happened
As the Grinder Operator was removing the old duct tape at the bottom of the chute he was standing on a two step mobile stair case. When he jerked the tape off his momentum caused him to go backwards and the stair case moved forward under him causing him to fall backwards.
