Employee was restocking packaging supplies on a tall shelf using a large yellow ladder

employee stated ' 'I was stocking supplies above commercial bakery in the produce backroom. I grabbed the tall yellow ladder and placed it below the racking and ensured the break was on. I then took a box of packaging up the ladder and went to place it on the racking. Suddenly the ladder moved and I fell to the left off the ladder platform and landed on another employee causing us both to fall to the floor. ' '
