Before the incident
EE alleges that [REDACTED] while in the warehouse putting product away she slid a box forward on the top shelf but there was an item loose in the box which came out and dropped from approximately 1.5-2ft above her head and hit her on her upper lip resulting in a cut on her upper lip that goes all the way through to the inside of her mouth. EE reports that the cut has been actively bleeding so she is applying continuous firm pressure at this time to stop the bleeding. EE states the item that fell was a metal seal fitting top that is heavy (unknown weight) and hit her squarely on her upper lip. EE states that she does not feel any injury to her teeth at this time but states that she has not been able to check for injury to her teeth yet. EE did not lose consciousness as a result of being hit in the face and states that she does not see any other cuts or scrapes on the rest of her face. EE referred to provider dt having a cut on her face.
