Patient became combative with nurse. Patient threatened to kill himself with call device cord when nurse tried to help unwrap cord from around patient. Patient grabbed nurse and twisted arm back and would not let go. Patient bent fingers back of nurse. Nurse hurt heel when falling back from grasp of patient.
