EE was dumping a hopper into an open top on the yard. EE stepped up on the forklift wheel and placed left hand between hopper and forklift forks. EE used the right hand to pull the hopper lever. When the hopper dumped it tilted back and trapped EE 's left hand between the forks and the hopper.
