What happened
The employee was stringing up machine #3. To string up the machine a wire pointer was needed. The employee went to the nearby wire pointer that was on casting wheels and started pulling it towards the machine. While pulling the wire pointer the casting wheel caught on a part of the concrete. The employee lost balance and the wire pointer fell onto the employee 's foot.
