At roughly [REDACTED] I was stepping out of the delivery van with packages in my hand I was walking up to the driveway on the curb at that point my left foot went into a storm drain and twisted which made me fall when I fell I tried to catch myself with my right hand which then it twisted inward and i fell on the driveway and hit my forehead on the right side on the driveway
