Before the incident
Loading a small roll of plastic film (24-inch wide 8-inch diameter about 25 lbs) onto slitter SO3.
What happened
He took the roll off the lift cart and manually placed the right end of the core onto the right-hand chuck then held the roll with his left hand while he reach with his right to press the button to bring the chucks in so they would grab the roll. The tip of his middle finger was in the path of the chuck and was crushed fracturing the fingertip.
