What happened
Forklift driver placed a master roll of plastic film (about 2000 lbs) on the floor behind her then nudged it with the forks in an attempt to move it to the position where he wanted it (he should exited the forklift and done this with the roll pusher device). The roll moved too far running into the back of [REDACTED] 's left leg.
