THE PROTABLE PLATE CAME UP AND THEN BACK DOWN TRAPPING HIS RT FOOT. THEN THE PALLET JACK CONTINUED TO ROLL UP ON TOP OF THE PORTABLE PLATE. HE FELL BACKWARDS AND HE GOT UP TO GRAB THE CONTROLS TO REMOVE THE PALLET JACK OFF THE PLATE AND OFF HIS FOOT. HE HAD MINOR [REDACTED] AND IT STARTED SWELLING. WHEN HE GOT HOME HIS RT FOOT AND ANKLE WERE SWOLLEN
