What happened
TM has been with the company since [REDACTED]. The TM has been fully trained in all policy and procedures. TM was wearing all required PPE. On [REDACTED] at approximately [REDACTED] at the [REDACTED] on the cold dock at dock 10 inside trailer 658087 TM was in the process of loading 13 ' ' boxes onto his assigned route trailer. As the TM reached the top of the stack (the 18th box on the stack just under the roof of the trailer). TM was standing on a wooden pallet and was attempted to jump and throw at the same time a bundle of 13 boxes to the top of the stack that he built. As the TM landed TM twisted his right ankle. Causing the TM to fall on the trailer floor.
