Adjusting the pipe wrench to fit the turnbuckle to hold back up

A crew working in reactor S [REDACTED] was tasked with lowering a vertical 60 inch riser pipe approximately 6 8 inches to make a fit up on the previously cut out portion of riser pipe below The riser pipe was inserted tightly into a sleeve above and secured by 3 engineered 1 inch cable and turnbuckle supports that were fastened to the shell of the reactor In an attempt to lower the 60 inch riser pipe the crew used rigging to relieve pressure on the turnbuckles to allow the turnbuckles to be loosened Two of the three turnbuckles were loosened without incident Tension remained on the third turnbuckle The crew then made the two loose turnbuckles taut to assist in distributing the weight of the riser pipe While the injured employee was preparing to hold back up with a pipe wrench on the third and final turnbuckle support the pipe wrench unexpectedly propelled forward striking the employee in the left side of his face causing injury
