What happened

Employee climbed to first elevation of structure E223 to retrieve tool bag It was his intention to use rope to lower his tool bag The rope he intended to used to tie tool bag was located on the south side of the module secured to a handrail In order to reach the rope the employee placed his foot on an 8 inch XXS pipe that was resting on the grating The pipe rolled and slid approximately 6 inches causing his foot to be caught between the 8 inch pipe and a steel pipe support This pipe support is welded to the steel At the time the employees foot was pinned under the 8 inch pipe Fellow employees used crane to lift pipe and free his foot
