The staff were switching the individuals from the van into the car given the low amount of gas in the van.

As the individual was being buckled the individual started to have a behavior that continued as the individual was taken into the house to de-escalate the situation. The staff had items thrown at her and was eventually shoved by the individual onto the items. At which point the staff also broke her nail and hit her head.

Trauma and pain of the hip and head.

During a behavior one of the individuals shoved the staff down onto a bunch of objects she had previously thrown. The staff broke her nails and hit her head during the fall. Xrays showed no serious injury only trauma and pain were cited as the diagnosis
