What happened
LOTO procedures were followed including engaging the motor brake. Technician 1 (Impacted Person) unbolted the motor assembly to replace it. To remove the motor the chain tension needed to be released. Technician 2 disconnected the master link on one side of the chain but tension remained on the right side due to the unsupported weight of the bucket lift which was 18 off the floor.
