Climbing Ladder Stairs

EE presents to OHO on [REDACTED] at [REDACTED] to follow up after injury on [REDACTED] at [REDACTED]. EE reports she was stepping off a 4 step step stool ladder and thought she was on the ground but actually had one more step to go and fell. EE states that she twisted her left knee and left ankle as she fell and bumped left parietal lobe of her head on the door. EE reports that she immediately felt pain to her left knee 5 10 and some pain to her left ankle 3-4 10. EE denied any serious head injury pain headache and only felt some discomfort in her head as she did not hit it hard on the door after her fall. EE denies any LOC upon falling. EE reported the injury to her supervisor and was treated in the ER. Assessment of left knee and left ankle completed. Xrays of the Left knee were completed along with CT scan of her left knee. See reports. No imaging was completed on her left ankle. Naproxen knee immobilizer and crutches given in ER. PSH: Carpal tunnel syndrome- surgery to right wrist.
