EE contacted via telemedicine today [REDACTED] [REDACTED] regarding WPV incident workers comp that occurred on [REDACTED] at [REDACTED]. EE states that a patient was brought into the [REDACTED] and was in a calm state. EE states that the patient then urinated on the floor and on their linens. After this occurred the EE had assistance from another RN and the house supervisor in cleaning up the patient and mopping the floor. EE states the patient then sat up quickly moved to the end of the stretcher and without warning lunged and closed fist punched him in the left upper cheek temple area with his right fist. EE states the patient then grabbed the back of his head and tried to knee him in the face but the EE was able to get away before this occurred. The EE states he did not have an LOC and can recall the entire situation. EE denies any blurry vision vomiting or other s s of concussion. EE reports PERRLA.
