What happened

[REDACTED] was building a mixed pallet in B aisle and had grabbed a stack of 5 pallets. He asked [REDACTED] who was close by if he had too many pallets for the amount of cases he had to grab in which [REDACTED] replied yes. [REDACTED] [REDACTED] was standing on his machine behind [REDACTED] waiting for him to grab the pallets he needed. As [REDACTED] was removing a couple of the pallets from the stack he grabbed he slipped and fell backwards against [REDACTED] pallets. [REDACTED] then came up to me and said he had to put a band-aid on his leg because he had brushed a pallet and scraped his leg. [REDACTED] said he was not paying attention when the incident occurred. He never told me the whole incident until the morning of the [REDACTED]. He apologized for not telling me the night [REDACTED] when I had asked what had happened. I documented what he originally said.
