To the knowledge of the management team this issue was a previously established condition no work injury was reported. Date of injury above was listed as today as there was not prior notice given. This should be considered a questionable incident given such. I noted the side of body as Left however I am unsure what side he actually had surgery on. All dates times listed regarding this injury are not exact as there was no report of injury at any time. This case should be considered suspicious as there was not notification of injury. We had the understanding that this was a previous issue that he had experienced for some time and had been receiving medical treatment for some time regarding this issue.
