What happened
Driver was standing helping guide the customer to load the extended length freight into the trailer. Customer forklift blade got wedged inside a metal piece on the dock. Customer tried to raise blades up not realizing his blade was stuck inside the metal piece. When the blade cleared the metal piece it popped the extensions up into the air and into our driver 's face.
