Stripping a wall form (12 'x16 ') as they lifted the form 2ft off the ground using the tower crane.
What happened
As employee was stripping the wall form the wall form shifted to the side and the 2x4 guard rail got caught into a wall forms guardrailpost resultnig on the metal guard rail to detach from the walkway bracket causing the guard rail post to fall 12ft and hit employee 's head. Employee was evaluated by Occupoints and recommended to go to Urgent Care. Employee was released without restrictions.
