Employee stepped into a puddle and there was a nail he did not see. So when he stepped into the puddle he stepped on a nail which punctured his left foot. He was seen at Concentra Urgent Care located at [REDACTED] where they cleaned his wound gave him a tetanus shot wrapped it up and was released from care. The physician sent him back to the jobsite.
