Attempting to place gait belt to walk patient back to bed from the bathroom

What happened

I was attempting to put a gait belt on the patient to walk him back to bed from the bathroom. This was immediately after reattaching his telemetry box to the cords. He grabbed my left hand and bent my fingers backwards pulled my badge off breaking the cord then grabbed my hair and pulled me downward and to the left. Sometime during this I was hit on the left cheekbone. When I was able I activated the staff assist alarm on the call box in the room.
