What happened
Had vaccinated [REDACTED] patient with a Dtap vaccine with her laying on the exam table. After injecting the patient and while holding the syringe in my right hand I was sliding the safety cap into position with my left hand when the cap slipped and grazed my left index finger causing a puncture that was evident when I removed my gloves. Had sanitized my hands prior to donning gloves.
