What happened
[REDACTED] had been intermittently responsive. Dr. along with several ED staff and security were present at bedside due to patient have several loud outbursts. Dr. was finishing patient 's exam. [REDACTED] was not currently yelling at the time. He looked over at Dr. looked over at [REDACTED] standing beside the bed grabbed her by the back of her head and by the neck and hit her in the chest. Multiple staff immediately responsive to incident and attempting to pull patient off of [REDACTED].
