What happened
When cutting the last 10 inch tube from one of the longer pieces the remaining tube was not long enough to pass through both openings so the operator squeezed their left hand through the 3 1 4 wide x 2 3 4 high guarding opening to get the pipe through to the other side and position end in clamp blocks. With left hand still inside employee cycled machine with foot pedal and blocks pinched tip of left thumb resulting in a laceration on the tip of left thumb
